Printed Application form submitted at the time of application submission. 2. 10th certificate for Age proof, Father’s Name and Mother’s Name. 3. 12th Mark sheet, certificate 4. Undergraduate mark sheet and certificate. (For MCA Admission). 5. Haryana domicile certificate (as per Annexure-B-4) (candidate whose qualifying degree is from outside Haryana and claims for Haryana resident). 6. SC category certificate (as per Annexure-B-2 and B-6) if applicable (only for Haryana, consider under reservation). 7. BC-A/BC-B category certificate (as per Annexure-B-7) if applicable (only for Haryana, consider under reservation). 8. Income certificate for BC-Non-Creamy layer (as per Annexure B-8). 9. Economical Weaker Section Certificate (if applicable). 10. Domicile certificate (in case of J&K Migrants). 11. Medical certificate for Physically Dis-abled candidates (As per Annexure-B-9)-if applicable 12. Medical fitness certificate (as per Annexure-B-15). 13.
